LCS is owned and operated by seasoned root-cause analyst Patrick Fortune.
Pat has worked in resource industries for over 30 years, starting in process operations (first in pulp & paper, then oil & gas) where he obtained his 2nd Class Power Engineering Certificate before moving to safety services.
In his roles over the past 20 years, Pat has investigated numerous, serious, workplace incidents until retiring from the industry in 2018. Now, Pat spends his time working with TapRoot® across Canada and the U.S., as well as contracting his root cause analysis skills to companies when incidents occur.
When not teaching courses with TapRooT®, or leading investigations, you can find Pat enjoying some of his less safety-oriented hobbies, such as paragliding, kite surfing, or sailing.
