At LCS, we believe effective safety starts by understanding how work is actually performed.
Policies, procedures, regulations, and training are important. But they only become effective when they influence decisions and actions in the field.

Our approach is built around five practical principles:

Before we try to improve a system, we need to understand how the work is really being done.

We look beyond written procedures to understand the people, processes, equipment, environment, decisions, pressures, and organizational conditions that influence performance.

When something goes wrong, stopping at “operator error” or “failure to follow procedure” is rarely enough.

LCS uses structured investigation and root cause analysis to understand why the conditions existed that allowed an incident to occur. The objective is not blame. The objective is learning and improvement.

A safety improvement is only valuable if it reduces risk in the real world.

We focus on corrective actions that address weaknesses in systems, processes, controls, leadership, training, equipment, and organizational decision-making, rather than simply adding another rule or reminder.

Meet Patrick Fortune
Founder & Director | Safety Professional | Speaker | Mentor

Patrick Fortune is an experienced safety professional, investigator, root cause analyst, and mentor whose career spans frontline operations, major projects, EHS leadership, serious incident investigation, and safety system improvement. His operational background, including control room operations, emergency response, power engineering, thermal recovery, and sour gas operations, gives him a practical understanding of the realities and pressures of high-risk work. He has held senior investigation and EHS roles with EnCana, Suncor, and Fort Hills, where he led serious incident investigations, facilitated root cause analysis, developed investigation protocols, and advised senior leaders on complex safety issues.

As a certified international TapRooT® instructor, Patrick combines technical investigation expertise with a strong commitment to developing people. His experience includes investigating fatalities, fires, explosions, serious injuries, process failures, mining and aviation events, and significant near misses across a wide range of industries. Through LCS, he provides investigation support, root cause analysis, training, and mentorship with one clear objective: to help organizations understand why failures occur, strengthen the systems behind their work, and develop the capability to become better at safety.
